Required Dependent Verification

In an effort to make sure our employee benefits are managed in compliance with our carrier contracts, we will require all current and future employees to provide proof that any dependents for whom you wish to cover are, in fact, eligible for coverage. This is an important step to ensure we are in compliance with our legal responsibility to pay benefits only to our health plan’s eligible participants.

Who is a legal dependent?

Dependent children are those for whom you have full-legal guardianship – partial guardianship does not meet eligibility requirements.

Required Documentation

If you wish to enroll a dependent child and/or spouse in any of our plans, you will need to provide the following documentation directly to Human Resources prior to completing enrollment.

Documentation need only be provided one time, and will be kept on file for future years. If in a future year you add a new dependent, proper documentation will be required.

Dependent child

  • Birth certificate
  • Hospital letter for newborns
  • Court documentation establishing full-legal guardianship
  • Adoption papers
  • Medical documentation of disability for dependents over age 26

Enrollment
Dependents for you whom we receive proper documentation will be enrolled as of your effective date. We are unable to enroll any dependents for any coverage without proper documentation.

If it is found that someone is enrolled and does not meet eligibility, that person will be terminated from the plan retro to the enrollment effective date of coverage and all claims will reprocessed and denied retro to the enrollment effective date.
