Short Term Disability

Short Term Disability insurance provides employees the opportunity to protect their income should they medically be unable to work due to pregnancy/childbirth, injury, or illness.

Short Term Disability benefits are based on 60 percent of the weekly salary, up to a $1250 maximum, of their base pay for up to 13 weeks in the event they are unable to work due to an illness or injury and they are under the care of a physician. There is a 7 day elimination period that must be satisfied before benefits may be paid on the plan.

 

Disability Claims through Principal STD or LTD

To initiate a Short Term or Long Term Disability Claim, or to request FMLA or leave of absence, employees will need to contact Principal Absence Management Center (PAMC) to initiate a case. PAMC will determine which applicable benefits and/or leave options will apply to the request and provide the appropriate paperwork or forms for employees to complete:

You will want to have the following information handy when you call:

  • Employer Name: National Seating & Mobility, Inc.
  • Your NSM Employee ID #
  • Reason for filing the claim
