The plan provides for a monthly benefit of 60 percent of base pay, up to $10,000 a month, subject to the definition of disability and other terms of the contract. There is a 90 day elimination period that employees must satisfy before disability benefits may be paid. The long term disability coverage is paid for entirely by NSM.
To initiate a Short Term or Long Term Disability Claim, or to request FMLA or leave of absence, employees will need to contact Principal Absence Management Center (PAMC) to initiate a case.
