Long Term Disability Coverage

Long-term disability coverage is provided to eligible employees who are medically unable to work due to injury or illness.

The plan provides for a monthly benefit of 60 percent of base pay, up to $10,000 a month, subject to the definition of disability and other terms of the contract. There is a 90 day elimination period that employees must satisfy before disability benefits may be paid. The long term disability coverage is paid for entirely by NSM.


Disability Claims through Principal STD or LTD

To initiate a Short Term or Long Term Disability Claim, or to request FMLA or leave of absence, employees will need to contact Principal Absence Management Center (PAMC) to initiate a case. PAMC will determine which applicable benefits and/or leave options will apply to the request and provide the appropriate paperwork or forms for employees to complete:

You will want to have the following information handy when you call:

  • Employer Name: National Seating & Mobility, Inc.
  • Your NSM Employee ID #
  • Reason for filing the claim
