As a full– or part-time employee of the City of Gardner, you have the choice between four medical plan options: a High Deductible Plan (HDHP) and three Preferred Provider Organizations (PPOs).

While all four plans give you the option of using out-of-network providers, you can save money by using in-network providers because Humana has negotiated significant discounts. If you choose to go out-of-network, you’ll be responsible for the difference between the actual charge and Humana’s UCR (Usual, Customary and Reasonable) charge, plus your out-of-network deductible and coinsurance.

The HDHP offers you significantly lower premiums than the other three plans, and you can establish a Health Savings Account (HSA) with HSA Bank and contribute all or part of the premium savings into the HSA. These funds can be used to cover medical expenses, including deductibles, and they’re yours forever — even if you leave the City of Gardner. And unlike a Flexible Spending Account (FSA), they are not forfeited at the end of each year.

Please note: All plan deductibles will run from July 1 – June 30th.
